ABSTRACT

Objective::To explore the effect of modified Longdan Xiegantang in treatment of anal sinusitis patients with syndrome of interior dampness-heat and on immune response, oxidative stress. Method::A total of 80 cases of anal sinusitis patients with syndrome of interior dampness-heat treated in our hospital from May 2017 to May 2018 were randomly divided into two groups, with 40 cases in each group. The control group was treated by enema with Metronidazole injection combined with gentamicin sulfate injection. In addition to the therapy of the control group, the treatment group was also given modified Longdan Xiegantang. The symptoms and signs score, interleukin (IL)-2, IL-4, IL-6, IL-10, tumor necrosis factor-α (TNF-α), interferon-γ (IFN-γ), procalcitonin (PCT), CD3+, CD4+, CD8+, immunoglobulin A(IgA), IgG, IgM, IgE, SOD, propylene glycol (MDA) levels of two groups were observed before and after treatment, and the curative effect, reactions and relapse were observed. Result::The clinical efficacy of treatment group was significantly better than that of control group, and the rank-sum test showed statistically significant differences (Z=1.981, P<0.05). After treatment, the scores of symptoms, such as anal distension and pain, and signs, such as anal sinus tenderness, hyperemia and edema in the treatment group were lower than those in control group (P<0.05). The levels of TNF-α, PCT, IL-2, IL-6 and MDA in treatment group were lower than those in control group (P<0.05). The levels of IL-4, IL-10, IFN-γ and SOD in treatment group were higher than those in control group (P<0.05). The recurrence rate was 5.13%in treatment group, and 21.05%in control group, indicating a significant difference (χ2=4.319, P<0.05). Conclusion::Modified Longdan Xiegantang is effective in treating syndrome of interior dampness-heat anal sinusitis. It can effectively relieve symptoms, regulate pro-inflammatory/anti-inflammatory balance and Th1/Th2 balance, improve cellular immune function and humoral immune function, inhibit oxidative stress response, and reduce recurrence rate, with a high safety.

ABSTRACT

Objective::To observe the clinical efficacy of Longdan Xiegantang in the treatment of pelvic inflammatory infertility with damp-heat phlegm and phlegm, in order to explore its mechanism of action. Method::Totally 150 patients with phlegm and inflammatory pelvic inflammatory infertility admitted to First Hospital of Hunan University of Chinese Medicine from January 2017 to December 2018 were enrolled in the study. According to the random number, the patients were divided into control group and observation group, with 75 cases in each group. Control group was given antibiotics combined with Gandan Shuangqing granule, and observation group was given Longdan Xiegantang combined with antibiotics. Both groups were treated for 6 weeks. The clinical efficacy, traditional Chinese medicine (TCM) syndrome scores, blood rheology and serum biochemical parameters, inflammatory factor levels and immune function indexes of the two groups were compared. Result::The total effective rate of observation group was 93.33%, which was higher than 78.67% of control group (χ2=6.480, P<0.05). The pregnancy rate of observation group was 58.33%, which was significantly higher than 33.82% of control group (χ2=8.862, P<0.01). Before treatment, there was no significant difference in TCM syndrome score, hemorheology, serum biochemical index, inflammatory factor level and immune function between the two groups. Compared with before treatment, the TCM syndrome scores of lower abdominal pain, anal bulge, lumbosacral pain, abnormal bandage and menstrual disorders were significantly reduced, the levels of plasma viscosity, hematocrit, high and low-shear whole blood viscosity were significantly reduced, the level of inhibin-A (INH-A) was significantly increased, the level of progesterone (P) and beita chorionic gonadotropin (β-HCG) were significantly reduced, the levels of serum C-reactive protein (CRP), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were significantly decreased, the levels of CD4+ and CD8+ were significantly increased, and the levels of IgA, IgG and IgM were significantly decreased (P<0.05). After treatment, in observation group, TCM syndrome scores of the lower abdominal pain, anal bulge, lumbosacral pain, abnormal bandage and menstrual disorders were significantly reduced, the levels of plasma viscosity, hematocrit, high and low-shear whole blood viscosity were significantly reduced, the level of INH-A was significantly increased, and the level of P and β-HCG were significantly reduced, the levels of serum CRP, IL-6, TNF-α were significantly decreased, the levels of CD4+ and CD8+ were significantly increased, and the levels of IgA, IgG and IgM were significantly decreased (P<0.05). There was no obvious adverse reaction in two groups during the treatment. Conclusion::Longdan Xiegantang has a good clinical effect in treating pelvic inflammatory infertility with damp-heat phlegm and phlegm, and can reduce TCM syndrome scores, and increase pregnancy rate. The mechanism may be correlated with the improvement of blood rheology and serum biochemical indicators, the inhibition of inflammatory factors and the enhancement of immune function.

ABSTRACT

Objective: To explore the effect of Longdan Xiegantang on serum inflammatory factors, related proteins and immune function in patients of secretory otitis media (SOM) with liver and gallbladder wetness-heat Syndrome. Method:Totally 76 cases of SOM with liver and gallbladder wetness-heat syndrome admitted to our hospital from July 2017 to May 2018 were randomly divided into two groups, with 38 cases in each group. Control group was treated with triamcinolone acetonide and ambroxol. In addition to the therapy of control group, observation group was also treated with Longdan Xiegantang. Immunoglobulin (Ig) A, IgG, IgM, CD3+, CD4+, CD8+and NK, interleukin-1 beta (IL-1β), IL-5, IL-8, tumor necrosis factor-α (TNF-α), platelet activating factor (PAF), calcitonin (PCT) and water channel protein-1 (AQP-1), AQP-4, fiber link protein (Fn) and soluble interleukin-2 receptor (SIL-2R) levels of two groups were observed before and after treatment. Curative effect and adverse reaction were observed. Result:①Curative effect, after treatment, the total effective rate of observation group was 92.11%, which was higher than 76.32% of control group, with statistically significant differences (Z=2.108, Pα, PAF, PCT, IL-1β and IL-8 in observation group were lower than those in control group after treatment (PPP+, IgA, IgG and IgM of observation group were lower than those of control group (P+, CD4+, CD4+/CD8+ and NK were higher than those of control group (PConclusion:Longdan Xiegantang has a remarkable effect in treating patients of secretory otitis media with liver and gallbladder wetness-heat syndrome, and can restore symptoms, inhibit inflammatory response, activate cell and humeral immune system, reduce the secretion of AQP-1, SIL-2R and other proteins, and increase the secretion of AQP-4 and Fn proteins.
